CHEC TALENT FAIR 2008

Individual Entry Form

1.  This special event is for children (ages 9 and up) of registered conference of attendees only.

     Please reigster before applying at www.chec.org.

2.  Each family member entering must complete a separate Entry Form.

3.  Label your piece with its title, your name, age, phone, address, and category number.

4.  Securely fasten this information to the back/underside of each and every entry piece.

5.  Entry forms must be postmarked by May 12, 2008. Do not mail projects to the CHEC

      office.

6.  Registered attendee will bring the entry to the Denver Merchandise Mart on Thursday,

     June 12 by noon unless other arrangements have been made with Terresa Hill.

7.  Entries must be picked up no earlier than 4:00 PM and no later than 6:00 PM on

     Saturday, June 14.

Participant/Parent Agreement

Please mail all your family’s entries together.

We understand that all projects will be displayed at the CHEC June Conference and that photo or video recordings in which we and/or our projects appear may be used by CHEC for future promotional purposes. We understand that although all possible care is taken with each entry, neither CHEC nor its volunteers will be held liable for damage or loss. We also agree to abide by all the guidelines in the General Information and Specific Requirement forms.
